Required information
- Identify the copyrighted work you claim is infringed.
- Provide the exact VelvetStream URL and enough information to locate the material.
- Provide your name, mailing address, telephone number, and email address.
- State your good-faith belief that the use is not authorized by the owner, agent, or law.
- State, under penalty of perjury, that the notice is accurate and you are authorized to act.
- Include your physical or electronic signature.
A monitored legal address must be configured before public launch. No removal notice should be sent until that address appears here.
Misrepresentation
Knowingly making a material misrepresentation in a removal notice or counter-notice may create liability. Consider obtaining legal advice.
